Reporting Mistreatment in Childbirth
RhodeIslandBirth.com is offering a survey for pregnant and birthing people, support people, doulas, nurses, providers, other staff and everyone who loves them. This survey can be filled out by anyone who was physically present and witnessed care in a Rhode Island birthing hospital. We would love to encourage full participation, all questions are made optional for your convenience. RhodeIslandBirth.com creates reports available to the public. These experiences guide our advocacy with the state, as well as education for families and birth workers to prevent harm.
This survey is in two parts, and should take 5-10 minutes to fill out:
-Page 1: Demographic information
-Page 2: Informed Consent Survey
